Medical records must be maintained by any Clinical Practitioner who provides medical and/or referral services to any Healthfirst member for a period of 10 years after the last visit date, or, in the case of minor children, for 10 years from the age of majority. For more information, see section 5.2 Medical Record Reviews and Documentation Standards in the Healthfirst Provider Manual.
