Healthfirst Member Rewards Forms

The Healthfirst Member Rewards Card Program is a way for Healthfirst members to earn rewards for getting needed care.

By completing preventive screenings and health initiatives - such as well-child visits, mammograms, and colorectal screenings - qualifying Healthfirst members can earn a Healthfirst Prepaid reward card to use at participating merchants.

 
Fill out one Rewards Form for each qualifying member. Form links are below.

Personal Wellness Plan (HARP)
