Somatus provides specialized kidney care management and telephonic disease management services on behalf of Healthfirst for eligible members diagnosed with CKD/ESRD.

As part of our continued effort to provide members with the best possible quality care, Healthfirst has partnered with Somatus Inc. (Somatus) to support members diagnosed with Chronic Kidney Disease (CKD) or End Stage Renal Disease (ESRD).

This partnership is designed to improve health outcomes and enhance members’ quality of life through care coordination and ongoing engagement with specialized renal nurse care managers.

What is Somatus?

Somatus provides specialized kidney care management and telephonic disease management services on behalf of Healthfirst for eligible members diagnosed with CKD/ESRD.

These clinical programs focus on evidence-based interventions and patient engagement to slow disease progression, reduce the need for dialysis, and support treatment options that improve outcomes and reduce complications.

What care management services do they provide?

Somatus’ CKD/ESRD Care Management program offers kidney disease consultation through a targeted case management approach consistent with the National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (KDOQI) guidelines.

What benefits does Somatus offer the provider?

Somatus’ specialty-trained renal nurse care managers can complement the provider’s existing treatment plan by:

  • Helping members better understand their condition and treatment options
  • Reinforcing clinical recommendations and care goals
  • Providing patient education and disease self-management support
Who is eligible for CKD/ESRD Care Management?

Healthfirst members may qualify for the Somatus program based on an analysis of their lab results, procedure codes, revenue codes, and diagnosis codes.

Once members are identified, Somatus will contact them to gauge their interest in receiving assistance.

What information might Somatus request from providers?

To support coordinated care, Somatus may contact providers to request relevant clinical information, such as:

  • Lab results
  • Patient Progression reports
  • Transplant information or options
  • Other information that may be beneficial to the patient’s needs (transportation, dietary education, etc.)

This information helps ensure alignment with members’ treatment goals and preferences.
