As part of Healthfirst’s ongoing commitment to regulatory compliance and high-quality care, all participating providers must complete their annual compliance requirements for 2025. These mandatory activities ensure adherence to state and federal standards and support our collective efforts in delivering culturally competent, coordinated care.
Please review each section carefully and complete the applicable training and/or attestations by December 31, 2025.

| FDR and Affiliate Compliance Attestation | First Tier, Downstream, and Related Entities (FDRs) and Affiliates that provide administrative or healthcare services to Healthfirst members are required to establish policies that comply with Healthfirst’s compliance and fraud, waste, and abuse requirements . FDR authorized representatives must thereafter submit an attestation by December 31, 2025. |
| Cultural Competency Training | Providers and staff who have regular and substantial contact with Healthfirst members in the following plans are required to complete the U.S. Department of Health & Human Services (HHS) Think Cultural Health training and submit an attestation by December 31, 2025. |
| OMIG Compliance Program Certification | Qualified providers must implement, maintain and attest to having an effective compliance program in accordance with New York State Office of the Medicaid Inspector General (OMIG) regulations under 18 NYCRR Part 521 by December 31, 2025 |
| Special Needs Plan (SNP) - Model of Care (MOC) Training | Providers and staff who have regular and substantial contact with Healthfirst members in the following plans are required to complete Healthfirst’s SNP Model of Care (MOC) Training by December 31, 2025. |
