Members with the following Healthfirst plans are eligible for recertification:

  • Child Health Plus
  • Essential Plan
  • Managed Long-Term Care
  • Medicaid
  • Medicare Advantage Plan with Medicaid or Medicare Savings Program
  • Personal Wellness Plan (HARP)
 

How to get started using the toolkit:
