Continuous Glucose Monitors for Diabetes Management

Healthfirst aims to enhance A1c control management, improve treatment outcomes, and prevent disease progression. We achieve these goals by first identifying Healthfirst plan members with Type 2 diabetes who are potentially eligible for a Continuous Glucose Monitor (CGM), after which we coordinate their care with providers to prevent future complications.

It addresses strategies to optimize meaningful and measurable health outcomes for individuals in populations impacted by COVID-19. Participants will consider the tools and resources that can address the opportunities, challenges, and impacts on managing the health of these New Yorkers. This includes policymaking; social, geographic, racial, and age-related factors; and structural components of the healthcare delivery system.
